Florida Licensed Transitional Care Coordinator

A company is looking for a Transitional Care Coordinator LPN.

Key Responsibilities

Oversee post-acute care utilization and monitor length of stay in skilled nursing facilities

Coordinate discharge planning and develop alternative plans for complicated cases

Participate in clinical rounds and conduct outreach for post-discharge follow-up

Required Qualifications

High School Graduate or equivalent

1 year of experience in skilled nursing, acute care, or post-acute care management

Licensed Practical Nurse (LPN) - State Licensure
