Project Duration : 14 weeks
Shift : Day Shift. 7 : 30am -4 : 30pm, Weekend Rotations
Job Description : Responsibilities :
The Registered Nurse (Non-Certified Case Manager) collaborates with physicians and multidisciplinary team members to develop and implement individualized care plans for assigned patients. This role ensures patients progress toward desired health outcomes through continuous assessment and monitoring of care. The RN identifies and resolves barriers to effective care and actively participates in the discharge planning process to ensure smooth transitions of care.
Minimum Qualifications :
Required :
- Current Montana Registered Nurse (RN) Licensure
- American Heart Association (AHA) Basic Life Support (BLS) Certification
- Minimum of 2 years of nursing experience
- Must have Utilization Review experience
Preferred :
Bachelor’s Degree in Nursing (BSN)2 years of acute care hospital experience as a Case ManagerEssential Job Functions :
Conducts comprehensive patient assessments within regulated timelines to determine case management needs based on clinical judgment and psychosocial factors.Develops and implements individualized case management plans in collaboration with the patient, caregivers, physicians, and other members of the healthcare team.Monitors and evaluates care plans for effectiveness, documenting interventions and outcomes, and making adjustments as needed.Manages an ongoing caseload, providing regular outreach and continued care coordination.Promotes integration of services, including behavioral health and long-term care, to ensure continuity and quality of care.Completes all required education, training, and compliance activities within designated deadlines per DNV and organizational policies.Knowledge, Skills, and Abilities :
