Sunbelt Staffing is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment in St. Louis, MO. This role requires proficiency in OASIS documentation and experience with Homecare Homebase. The RN will deliver high-quality patient care, including chronic condition management, wound care, and providing education to patients and families in a home health setting.

Key Responsibilities :

• Perform patient assessments and complete OASIS documentation
• Create and implement individualized care plans for each patient
• Provide skilled nursing services such as wound care, medication management, and patient education
• Collaborate with a multidisciplinary team to ensure optimal care delivery
• Educate patients and their families on disease management and health promotion
