Position : Community Health Worker
Location : Tulsa, OK (1) & Oklahoma City, OK (1)
Duration : 6-month Contract
Start Date : ASAP
The Community Health Worker (CHW) position will actively engage health plan members
and clinical contacts to increase their engagement with and increase their utilization of
their health plan benefits and offerings. The CHW is expected to serve as a field-based
resource, meeting with members in direct outreach in their homes and communities and
representing the organization in a positive and professional manner.
Primary Responsibilities
- Engage members at their residential homes, senior living apartments or within
assisted living communities
Provide member education regarding programs and their benefit plan offeringsEngage members to schedule their annual Optum at Home APC visit, as well asappointments with other clinicians, care team members, and / or programs based on
member need
Build member trust and foster their relationship with Optum health care providersActively engage with the member to remove any obstacles hindering or preventinghem from utilizing their health plan benefitsProactively address any challenges the member may encounter in the future toimprove their experience and increase their utilization of benefits and provider
engagement, escalate concerns and barriers as appropriate in support of the member
Attempt to complete members’ Health Risk Assessment, Social Determinants ofHealth assessment, identify other health plan needs and provide communityresources
Complete the Bone density screening-Osteoporosis Management in Women who hada fracture (OMW) for members as appropriate
Utilize internal electronic medical record system to document member engagementattempts and ensure improved coding accuracy
Plan routes for best time management of schedule and travel time / distance tooptimize engagement of identified members
Utilize scheduling tools to schedule members for various types of visits; ultimatelyresulting in an increased Stars gap closure and reducing medical expenses
Required Qualifications
High School Diploma / GED (or higher)3+ years of experience working within a community health setting or community outreach experienceAbility to work with individuals and / or a team with diverse backgrounds and experiencesAble to exhibit good judgement for improved patient outcomes with the ability to consult leadership and escalate as appropriateRequired to have a dedicated work area established that is separated from other living areas and provides information privacyAbility to keep all company sensitive documents secureMust live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet serviceIntermediate level of computer experience with the ability to navigate a Windows environmentAccess to reliable transportation enabling you to travel to member homes and communities within a designated areaAbility to travel for field-based work, valid driver’s licenseMust be able to plan routes for best time management of schedule and travel time / distanceAbility to have flexibility with scheduled hours based on member’s availability and / or needsPreferred Qualifications
3+ years' experience working with vulnerable populationsExperience working in salesLicensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, orMedical AssistantBilingual fluency in Spanish and EnglishCHW CertificationExperience working in a managed care organizationKnowledge of Medicare / Medicaid populationTelecommuter / work from home experience
