As a Field-Based RN Case Manager with UnitedHealth Group, you'll wear many hats and work in a variety of environments.

This is a Field Based position with a Home-Based office.

If you are located in Southeast Houston, TX market you will have the flexibility to work a Hybrid / Field schedule as you take on some tough challenges.

Primary Responsibilities :

• Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
• Support members with condition education, medication reviews, and connections to resources such as Provider Services, Private Duty Nursing, DME, and supplies
• Assist members with the transition from a care facility back to their home
• This role involves approximately 75% travel, and you'll need to be comfortable dealing with a full range of in-home and facility-based environments
• Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential
• This function is for UHC C&S Only (with the exception of the Community Health Worker role)
• Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities
