Licensed Practical Nurse - DH Family Practice - Full-Time - Days

Job Description :

• PURPOSE OF JOB : a nonexempt position responsible for performing a variety of duties depending on assignment. These duties include, but are not limited to, direct patient care, triaging phone calls, patient outreach, care coordination, clerical, environmental and organizational tasks.
• ESSENTIAL FUNCTIONS :
• Assesses, plans, implements, documents, coordinates, and manages patient care at the direction of the provider giving consideration to age-specific accommodations.
• Communicates and collaborates with patients, providers, family members and other health care disciplines in order to achieve excellence in patient care.
• Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and OSHA Patient Safety Goals.
• Actively participates in and supports departmental and organizational quality initiatives and goals.
• Maintains clinical competency.
• Other duties as assigned.
