About The Position

Roles & Responsibilities – LCSW

Provide counseling, crisis intervention, and psychotherapy to individuals, families, and groups.

Conduct psychosocial assessments and develop individualized care plans.

Collaborate with physicians, nurses, and interdisciplinary teams to address patient needs.

Support patients dealing with mental health issues, chronic illness, trauma, grief, or life transitions.

Assist patients in accessing community resources, financial aid, housing, or rehabilitation programs.

Maintain accurate and confidential patient records in compliance with HIPAA and organizational standards.

Participate in discharge planning, ensuring continuity of care after hospitalization or treatment.

Provide education on coping strategies, stress management, and mental health awareness.

Adhere to state licensing requirements and professional code of ethics.
