Status : Part time or Full-Time

Visit rates : $35 - $40 per visit, plus mileage and travel time compensation

Position : Home Health Licensed Practical Nurse (LPN)

Your responsibilities as a Home Health LPN will include :

  • Delivering home-based care to patients with various medical conditions post-hospitalization, under the supervision of an RN.
  • Collaborating with a multidisciplinary team of healthcare professionals to ensure quality patient care.
  • Monitoring patient vitals, conducting assessments, documenting progress, and reporting changes to your RN supervisor.
  • Assisting the RN with implementing the patient's care plan and teaching self-care techniques to patients.
  • Providing education to family members on care plans, medications, and home safety practices.
  • Working with the healthcare team to coordinate care and facilitate effective discharge planning.

    • Qualifications we are looking for :

  • Graduation from an accredited practical nursing program and a valid LPN license in Colorado.
  • A minimum of six months of LPN experience, preferably in home healthcare.
  • Wound care and / or Medical-Surgical experience is advantageous.
  • Current CPR Certification is required.
  • Familiarity with state and federal home health regulations.
  • Strong clinical nursing skills, effective communication ability, and a compassionate demeanor.
