Job Summary and Qualifications
As a Registered Nurse dedicated to Discharge Coordination, your primary focus is delivering top-notch, patient-centered care. You will facilitate a smooth transition for patients entering and exiting our acute care hospital. Your specialized assessment skills will ensure patients are placed in appropriate care areas and receive essential post-hospitalization education. Advocating for patients, families, and caregivers while embodying our organization’s mission and values is central to this role.
What You Will Do in This Role :
Obtain nursing assessments and histories to ensure proper admissions to inpatient care units.Complete essential documentation, including admission assessments, medication reconciliation, and discharge paperwork.Educate patients, families, and caregivers on medical conditions, treatment plans, and follow-up measures in understandable terms.Administer discharge documentation and care for specialized patient populations to meet compliance standards.Provide necessary discharge paperwork and materials to patients and families.Collaborate with nursing administration to prioritize workload and ensure smooth transitions.Exhibit effective time management and the ability to prioritize tasks efficiently.Qualifications :
Required Experience : At least one year of RN experience.Preferred Experience : One year of hospital nursing experience.Required Education : Bachelor of Science in Nursing or enrollment in a BSN program to be completed within five years of hire. RNs with ten years or more of experience are exempt.Preferred Education : Master’s degree in Nursing.Required License : Current RN licensure with the North Carolina Board of Nursing. eNLC license may apply; please confirm with NCBON.Required Certification : BLS Healthcare Provider certification.Preferred Certifications : National certification in the area of specialty.Mission Hospital, part of Mission Health and HCA Healthcare, serves as the regional care center in Western North Carolina.
