Ohio Licensed Clinical Outcomes Manager

A company is looking for a Clinical Outcomes Manager - Population Health.

Key Responsibilities

Conducts regular in-person and virtual rounds to provide status reports and insights to providers and practices

Manages clinical outcomes, patient panel management, and performance metrics for physicians in the assigned market

Collaborates with providers to implement evidence-based protocols and supports process improvement initiatives

Required Qualifications, Training, and Education

Current RN Licensure accredited by relevant nursing education commissions (preferred)

BSN in Nursing or related Healthcare field (required)

MSN, MBA, or Master's degree in related Healthcare field (preferred)

5 years of experience in a healthcare setting with management or leadership experience in Population Health or Value-Based Contract work (required)

Working knowledge of process improvement models such as PDSA (preferred)
