Job Summary :
The Physical Therapist is responsible for delivering individualized physical therapy services including assessment, treatment planning, implementation, education, and communication to help patients achieve their functional goals. The therapist works collaboratively with other healthcare professionals and ancillary staff to ensure high-quality patient care and optimal rehabilitation outcomes.
Minimum Requirements :
Education :
- Bachelor’s degree in Physical Therapy from a program accredited by the American Physical Therapy Association (APTA) Commission on Accreditation in Physical Therapy Education
Experience :
Minimum of one (1) year of experience as a Physical Therapist in a supervised clinical setting within the last three yearsLicensure :
Current and valid Physical Therapist license to practice in the stateCPR certification preferred (may be required by facility)Essential Functions :
Provide direct patient care and evaluate functional needs and outcomesDevelop, implement, and revise individualized plans of care based on assessments and patient progressConsult and collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive treatment plansDirect the activities of physical therapy support staff to help patients achieve rehabilitation goalsEducate patients and their families on treatment plans, techniques, and home exercisesEnsure accurate and timely documentation of assessments, treatment goals, progress notes, and discharge plansParticipate in discharge planning and make appropriate recommendationsMaintain professional standards of practice and comply with state, federal, and facility policies and regulationsStay current with trends, research, and best practices in the field of physical therapyAssist in community outreach and education regarding physical therapy servicesOrganizational Expectations :
