Key Responsibilities

• Conduct comprehensive physical therapy assessments to evaluate patients' mobility, strength, range of motion, and functional abilities
• Develop and implement individualized treatment plans tailored to each patient's specific rehabilitation goals and medical conditions
• Deliver evidence-based physical therapy interventions including therapeutic exercises, manual therapy, gait training, and mobility assistance
• Provide patient and family education on home exercise programs, proper body mechanics, fall prevention, and adaptive equipment use
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare team members to ensure coordinated care
• Maintain detailed, accurate documentation of patient progress, treatment plans, and outcomes in compliance with regulatory standards
• Monitor patient progress and adjust treatment protocols as needed to optimize rehabilitation outcomes
• Ensure patient safety and infection control practices during all home visits
