Provider MUST be able to do a minimum of two area's listed below : Mammo / Breast Biopsy together are fineIR & DX or IR & Fluoroor IR & cardiac or anything with Mammo / Biopsy will be almost an auto approval. Must be able to do procedures. Must be willing to help with IR and / or DX call unless they are Mammo / Biopsy Comment which group they are able to do.Must have modality sheet uploadedMUST be enrolled in PCF Please note that every bid MUST have the following information listed on each bid.-Phone number-Active License Number-Active state DEA Number-Active CSR -The board verification-Email-Address (provider must be 90+ miles from facility)-DO / MD-DOBIf your bid is accepted, you must submit the credentialing documents by the deadline given by the onboarding coordinator. All credentialing documents are inthe "other documents" tab.

• Provider must be contracted with the agency prior to placing a bid.
• PLEASE NOTE : There will be no OT rate for this assignment request
• If your bid is accepted, you must submit the credentialing documents 7 daysafter acceptance as this is a new start for CHS HBP. Your bid runs the risk ofcancelation if all onboarding documents are not uploaded within the 7-daytimeframe. All credentialing documents are in the "other documents"tab.
