Location : Great Neck, NY area

Job Overview :

HealthYes is seeking a compassionate and experienced Family Nurse Practitioner in the Great Neck area, NY. This position focuses on delivering patient-centered care, managing acute and chronic conditions, and promoting health and wellness within a collaborative healthcare environment.

Key Responsibilities :

Conduct comprehensive patient assessments, including health histories and physical examinations.
Diagnose and manage acute and chronic illnesses, developing personalized treatment plans.
Address preventative care needs and close care gaps for assigned patients.
Order and interpret diagnostic tests and lab results; manage follow-ups.
Collaborate with healthcare professionals and community resources to ensure holistic care.
Participate in quality improvement initiatives and departmental committees.
Provide supervision and training to medical staff, including students and residents.

Qualifications :

Master of Nursing from an accredited institution.

Active New York State Family Nurse Practitioner license.

Spanish or Mandarin speaker preferred.

Experience Requirements :

2-3 years of experience as a practicing Family Nurse Practitioner (FNP) preferred.

Familiarity with medical devices (e.g., glucometers, blood pressure monitors).

Experience with community resource facilitation and referrals is advantageous.
