As the Admissions RN, you will be responsible for performing admissions at a patient's home, skilled nursing facility, or assisted livings. This role offers $73K-$93K annually based on experience! What You'll Do :

• Visit patients / families, physicians / health care professionals in hospitals and other settings to facilitate admission to hospice program
• Organize discharge plan for hospitalized patients
• Document in EMR in concise and timely manner per standards
• Follow OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biologicals
• Collaborate with Referral Center staff on an on-going basis
• Inpatient evaluations.
