JOB DESCRIPTION

Patients typically have multiple comorbidities and can include CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.

EPIC EMR charting, Sapphire IVs.

Any solid color scrub acceptable. Traveler must have own transportation to / from all patient visits.

Candidate MUST HAVE :

• 1 year Home Health RN experience
• active BLS (AHA) and state RN license (IL)
• wound care experience (wound vacs, pleural drains, ostomy care)
• able to do Peripheral blood draws, central line care, IV admin through elastomeric, IV pumps, etc.
• Excellent assessment skills
• Experience with computer documentation
