Licensed Practical Nurse - Brooklyn, NY (Onsite) - Full-Time / Perm Mon-Fri - 08 : 30am-04 : 30pm, 40 hours / week - $35.00 - $36.00 Per HR

Summary :

A Federally Qualified Health Center in Brooklyn has an immediate need for a Licensed Practical Nurse (LPN).
The LPN will provide direct patient care, perform injections and support medical assistant sin delivering high-quality care.

Key Responsibilities :

Prepare daily assignments for nursing staff and support clinical operations

Collaborate with the Director of Nursing to implement effective day-to-day systems

Participate as a member of the Patient-Centered Medical Home (PCMH) care team

Attend daily huddles and practice meetings, ensuring staff compliance with PCMH initiatives

Provide hands-on care with professionalism and empathy

Requirements :

NYS LPN license and Registration Certificate

BLS Certification

Strong communication and teamwork skills

Commitment to patient-centered care
