Job Description & Requirements Specialty : Long Term Care Discipline : RN Start Date : ASAP Duration : 7 weeks 36 hours per week Shift : 12 hours, nights Employment Type : Travel Job Title : Long-Term Care (LTC) Registered Nurse (RN) Job Summary : A Long-Term Care (LTC) Registered Nurse (RN) provides comprehensive, patient-centered care to individuals in long-term care facilities, such as nursing homes, rehabilitation centers, and assisted living communities. They are responsible for managing the overall health and well-being of patients, ensuring personalized care plans, and collaborating with interdisciplinary teams to promote optimal outcomes. Key Responsibilities : Provide direct nursing care to residents in accordance with individualized care plans. Administer medications, perform treatments, and monitor patients’ progress, ensuring safety and adherence to established protocols. Assess, plan, implement, and evaluate nursing care for long-term care residents. Collaborate with physicians, therapists, and other healthcare professionals to develop and adjust patient care plans. Monitor and document residents’ health status, including vital signs, medication administration, and overall progress. Educate residents and their families on health conditions, care needs, and treatment options. Respond to and manage acute and chronic conditions, ensuring timely interventions and care coordination. Provide emotional support and counseling to residents and their families, promoting a positive living environment. Ensure compliance with all federal, state, and facility regulations related to long-term care. Supervise and delegate tasks to licensed practical nurses (LPNs) and nursing assistants (CNAs). Participate in quality improvement initiatives, staff training, and continuing education. Maintain accurate patient records and documentation, adhering to established clinical standards and protocols. Address concerns related to infection control, safety, and preventative care to minimize risks to residents. Qualifications : Education : Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted. Licensure : Valid Registered Nurse (RN) license in the state of practice. Certification : Basic Life Support (BLS) certification required. Advanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.
