Core Responsibilities :

As an LPN, you are central to providing safe, effective, and resident-focused care. Your critical responsibilities include :

  • Clinical Documentation : Maintaining accurate and thorough Daily Documentation of all patient care, observations, and treatments in the resident record.
  • Order Management : Precisely Transcribe physicians’ orders to MARs (Medication Administration Records) and promptly Notify pharmacy of new orders and changes to ensure seamless pharmaceutical care.
  • Medication Administration : Prepare and safely administer medications as ordered by the physician, strictly adhering to all facility protocols and best practices.
  • Patient Monitoring : Proactively observe residents for any subtle or significant changes in condition, taking vital signs, and escalating concerns promptly to the Registered Nurse or Unit Manager.
  • Treatment Implementation : Skillfully perform and document various treatments, wound care, and procedures within your licensed scope of practice.
  • Supervision : Provide guidance and direction to Certified Nursing Assistants (CNAs) to ensure high standards of direct care delivery on your unit.
  • Perform all other duties as assigned by the Director of Nursing or Administrator, contributing to the smooth operation of the facility.
  • Qualifications :

  • Must possess an active and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin.
  • Demonstrated ability to work effectively and collaboratively within a team.
  • Previous experience in a long-term care or skilled nursing setting is preferred, but highly motivated new licensees are encouraged to apply.
