About the Role :

Are you passionate about making a real difference in the lives of families in need? As a Care Coordinator (Wraparound Facilitator), you'll be at the heart of our mission, helping adolescents (ages 5-21) and their families navigate challenges and build stronger, healthier futures. Your role will involve building and working closely with a team to create personalized support plans and coordinate a range of services that make a meaningful impact.

What You'll Do :

• Connect with Families and Team Members : Reach out to new families within 3 days and meet with them face-to-face within 7 days to start building strong relationships.
• Lead the Way : Build a team of supports and facilitate monthly Child and Family Team Meetings (CFTMs), keeping everyone updated with an evolving Plan of Care.
• Create and Customize : Develop and monitor personalized Plans of Care, ensuring they meet each family's unique needs and goals.
• Coordinate with Care : Make sure families get the educational, social, and mental health support they need, and ensure these services are high-quality and accessible.
• Keep It All on Track : Maintain detailed and timely records, including updates in our Electronic Health Record System.
• Safeguard Privacy : Handle all information with the utmost confidentiality, adhering to HIPAA, FERPA, and other privacy standards.

About Ascent :

Ascent Monroe is a nonprofit organization offering Wraparound Services to the following Parishes : Caldwell, East Carroll, Franklin, Lincoln, Madison, Morehouse, Ouachita, Richland, Tensas, Union, and West Carroll. The wraparound process is an intensive, individualized care management process for youths with serious or complex needs. Research shows that the wraparound process does more for children than basic child welfare services alone. According to numerous independent studies, most youths in welfare programs that interacted with Ascent move on to less restrictive environments and are significantly less likely to engage in illegal or delinquent behavior.
