RN MDS Nurse – Leadership Opportunity in the Caring Community of St. Leonard

What You’ll Bring :

1–2+ years MDS / RAI experience in SNF or LTC

Proficiency with PDPM, Section GG, and care plan coordination

PointClickCare (PCC) experience preferred

Active RN license required
