Key Essential Functions and Accountabilities of the Job

Leadership & Team Management

• Directly manage a team of managers or specialists responsible for handling referrals.
• Provide coaching, performance evaluations, and onboarding of new staff.
• Cultivate a high-performing, collaborative team culture.
• Maintain leadership accountability for ensuring timely and effective referrals.
• Establish a regular cadence for team members to work referrals for each panel and report back to PCP / Medical Home Teams with data and analytics.
• Engage PCPs to adhere to standard turnaround times for referrals.
