  • Job Summary :

We are seeking a compassionate and dedicated Hospice Licensed Vocational Nurse to join our team. The ideal candidate will provide high-quality care and support to terminally ill patients in their homes or in hospice facilities.

  • Qualifications :
  • Active Licensed Vocational Nurse (LVN) license in the state of practice.
  • Previous experience in hospice or palliative care is preferred.
  • Strong communication and interpersonal skills.
  • Compassionate and empathetic approach to patient care.
  • Ability to work effectively in a team environment.
  • Basic computer skills.
  • Responsibilities :
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and assess patient's condition and report changes to the healthcare team.
  • Provide emotional support and education to patients and their families.
  • Ensure all documentation is completed accurately and in a timely manner.
  • Maintain patient confidentiality and privacy at all times.
  • Follow all regulatory standards and company policies.
