Key Responsibilities

Maintain a caseload of patients and manage their enrollment in Complex Case Management programs

Develop and implement patient-centered care plans while collaborating with healthcare professionals

Identify and connect patients with community resources and assist in advanced care planning

Required Qualifications

Bachelor's Degree in Social Work (required); Master's Degree preferred

Licensure as required by state of practice (required)

2-3 years of experience in acute care, home health, or case management

Case Management certification, LSW, or LCSW preferred

Familiarity with community resources and healthcare practices
