Position Overview

The Wellness Nurse plays a pivotal role in supporting the health, wellness, and independence of our independent living residents. This role emphasizes relationship-centered care, proactive wellness strategies, and effective coordination with residents, families, and the care team to achieve the best possible outcomes.

Key Responsibilities

• Conduct comprehensive wellness assessments to promote holistic well-being.
• Coordinate services and interventions to help residents maintain their independence and enhance quality of life.
• Lead and participate in collaborative care meetings to promote communication and integrated care planning.
• Identify residents at risk and implement targeted strategies to mitigate those risks.
• Serve as a trusted resource and advocate, educating residents and families on health conditions, disease management, and wellness promotion.
• Develop, coordinate, and deliver wellness-focused programs, including health education and disease prevention initiatives

Qualifications

Education / Licensure:

• Active RN or LPN license in FL(required)

Experience:

• 2–3 years of experience in long-term care, post-acute care, or a similar healthcare setting (preferred)

Skills and Attributes:

• Strong clinical assessment and critical thinking abilities
• Excellent verbal and written communication skills
• A compassionate, resident-centered approach and the ability to build trust and rapport
• Effective organizational and time management skills
