Job descriptionAssessment of referred patients that require rehabilitation services including : providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. Setting realistic and achievable goals for their patients Documenting and recording the patient's condition Educating patients and families in an appropriate therapy method Graduate of an accredited Occupational Therapy program NJ Occupational Therapist (OT) license Experience with Long-Term Care and / or Geriatric Care Well versed with documentation writing
A healthcare network in Morris County, NJ is currently seeking a licensed Physical Therapist (PT) to join their growing team.
About the Opportunity :
- Start Date : ASAP
- Assignment Length : Ongoing (Contract to Hire)
- Schedule : 3 Days Per Week
- Hours : 8am to 5pm
- Patient Population : Adults / Geriatrics
- Patient Volume : 6-10 patients per day
- Setting : Skilled Nursing Facility
- EMR : PCC
