Social Worker

A nursing home in Nashville is seeking a Social Worker to join their team.

Social Worker Job Description:

• Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Social Worker Qualifications/Requirements:

• BSW or MSW preferred, or Bachelors degree in the related field
• At least 3 years experience in the Social Work field required
• Ability to work flexible schedule
• PCC experience preferred
