POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS - AZ / compact RN license

BLS

3 years of performing Case Management / Care Coordination in an acute hospital setting within the last 5 years

Experience with and proficient at Midas, InterQual, and Microsoft Office programs

Experience with discharge planning AND utilization review

Minimum of 3 years hospital experience

Recent experience with Interqual (within past 6 months)

Experience working with an EHR (electronic health record)

PREFERRED QUALIFICATIONS - Experience performing CM / CC in the Emergency Department

LENGTH OF ASSIGNMENT - 13 Weeks

SHIFT / HOURS PER WEEK - 0830-1700 plus every other weekend

SYSTEMS - Cerner

START DATE - ASAP
