Job Summary and Qualifications

The Physical Therapist Clinical Specialist functions as an interdisciplinary team member providing clinical expertise in area of specialty, direct patient care, assessments, and individualized care planning / implementation and oversight of support staff. Provides in-servicing and education to fellow colleagues, fosters an environment conducive for learning and mentorship, participates in program development and implementation of evidence-based practices based on outcomes and analysis and champions change management.

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient's physical therapy to support patients throughout the continuum of care.

We also offer urology, gastroenterology, obstetrics, and pulmonary and rehabilitative medicine.
