Job Summary :
The Licensed Practical Nurse (LPN) provides basic nursing care to residents under the direction of a Registered Nurse (RN) or physician. The LPN is responsible for monitoring patients’ health, administering medications, providing wound care, documenting changes, and ensuring residents receive compassionate, high-quality care in accordance with nursing home policies and state regulations.

Key Responsibilities :
Monitor residents’ vital signs and general health condition
Administer medications and treatments as prescribed
Provide wound care and assist with personal hygiene tasks
Observe and report any changes in residents’ physical or emotional condition
Maintain accurate and timely nursing documentation and medical records
Collaborate with RNs, CNAs, physicians, and family members on patient care plans
Support and supervise Certified Nursing Assistants (CNAs)
Follow infection control, safety, and HIPAA protocols
Assist during emergency situations and respond to resident calls promptly
Participate in care plan meetings and resident assessments
All other duties as assigned.
Qualifications :
Licensure : Valid and active LPN license in Ohio
Experience : Prior experience in long-term care or geriatric nursing preferred
CPR Certification : Required
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate, patient, and respectful of elderly residents
Physical Requirements :
Able to stand, walk, bend, lift (up to 50 lbs), and assist with transfers
Ability to work in a fast-paced environment with frequent resident interaction
