Reports to: Manager of Outreach & Case Management Services

Position Summary:

Provides comprehensive support services to homeless and indigent clients determined to have higher acuity and vulnerability due to high medical needs, including assessment, case planning, and evaluation of services to assist while enrolled in OBFH’s residential Health 2 Housing medical respite care with a focus on medical recovery and access to supportive housing.

Primary Responsibilities:

  • Advocates for the mission of the organization and promotes the vision and direction of the agency’s values
  • Delivers services with safety, respect, and effectiveness
  • Follows all policies & procedures of the organization
  • Works with referral sources to ensure continuity of care and compliance to funding source requirements
  • Upon admission, gathers baseline data concerning client’s use of medical services and housing prioritization needs
  • Participates in bimonthly case conferencing and rounds on-site with all involved team members
  • Provides and arranges referrals to appropriate service providers within the agency and externally
  • Balances and maintain a caseload of six (6) or more clients based on acuity, length of time in program, status of housing stability, etc.
  • Advocates for clients as needed
  • Provides hands-on modeling and teaching to assist clients with developing daily living skills (cleaning, cooking, meal planning, shopping, budgeting, etc.).
  • Reviews and evaluates client progress on an ongoing basis and adjusts care plans as appropriate
  • Organizes, documents, and maintains client case files/documentation in a timely fashion in accordance with policies & procedures and professional practice
  • Employees will set up, maintain and use daily the following communication tools to keep abreast of agency updates: agency voicemail and agency email.
  • In the provision of above services, the employee will utilize HCCH HOPE Team Outreach support and approved HCCH/OBFH rideshare resources to coordinate transportation of clients based on priority service needs. Only agency approved vehicles will be used by authorized employees to transport clients in accordance with agency transportation policy.
  • Performs other duties as assigned

Requirements:

  • High School Diploma, GED or equivalent experience (Bachelor’s degree in human services and/or certification in Case Management with commensurate experience preferred)
  • At least two years’ experience in Human Services field providing supportive case management services to at-risk, indigent, and/or homeless population (advocacy, housing placement, etc.)

Knowledge, Skills and Abilities:

  • Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, development delays, serious mental illness, traumatic brain injuries, literacy issues, and/or other conditions or situations that have impacted housing stability
  • Ability to work in a fast-paced environment
  • Ability to be flexible and able to multi-task
  • Must be solution-focused and organized in achieving objectives with all service users
  • Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments
  • Ability to maintain confidentiality and follow all federal and state HIPAA laws and regulations
  • Ability to handle stress and high pace of crisis intervention and follow protocols for safety and security
  • Excellent oral and written communication skills, including use of phones, in-person assistance, computers, fax, and other office machinery
  • Computer proficient and experience with EHR (Electronic Health Record) documentation preferred
  • Experience with HMIS (Homeless Management Information System) preferred

Additional Requirements:

  • Sitting, walking, lifting to 20 pounds, reaching, carrying, listening, speaking (Physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.)
  • Valid Driver’s License
  • Valid car insurance

Desired:

  • Bilingual or ASL ability a plus.

Some travel will be required.

Work Schedule: Monday through Friday (2-3 hours per day) Not to exceed 15hrs week
