About The Position

Job Title : Certified Occupational Therapy Assistant

Profession : Rehabilitation Therapy

Shift : Monday to Friday

Hours per Shift : 32

Experience : New graduates are acceptable. SNF experience preferred.

License : Must hold a valid state license.

Certifications : Valid CPR certification preferred.

Must-Have : Candidates must ensure the prescreen is filled out and attached upon submission.

Candidates must attach their resume and license verification.

If these items are not completed and attached, the candidate will be declined.

Covid test required 72 hours prior to start date with results issued 48 hours prior to start date.

Description :

Candidates must review the credentialing checklist prior to submission.

State license verification is required.

Resume and three references are needed.

County criminal background check for seven years for all counties lived and worked is required.

Federal criminal background check for seven years or more is mandatory.

Multi-state criminal search covering seven years or more is required.

Nationwide sex offender search covering seven years or more is necessary.

HHS / OIG search must be performed.

GSA search must be performed.

Social security number trace is necessary.

A two-step PPD or Quantiferon or proof of a positive PPD and chest X-ray must be provided.

SAM search is required.

Oregon state background check must be completed.
