In this role, you will be responsible for assessing, inserting, and managing various vascular access devices, including PICC lines, midlines, and ultrasound-guided peripheral IVs. who excels in this role : \nAccurately performs patient assessments and identifies patient needs\nIdentifies and initiates appropriate nursing interventions\nProvides care appropriate to condition and age of the patient\nPerforms timely and appropriate documentation relating to medical necessity in the medical record\nResponsible for completion and revision of the Interdisciplinary Care Plan for each patient\nPerforms timely and accurate QI assessments\nAssesses patients for the need for a mid-to-long-term vascular access device (VAD) to provide a reliable device for therapy utilizing evidence-based guidelines for VAD placement and care.\nAssesses patients with difficult intravenous access (DIVA) for appropriate VAD need\nAssesses patient information via electronic chart reviews for indications, pertinent history, and lab values and medication administration record\nVerifies indications for VAD placement, coordinate with physicians and provide recommendations for selection of appropriate vascular access device\nCoordinates discharge planning and post-discharge follow-up for VAD's, in collaboration with inpatient hospitalists\nProvides vascular access device placement for patients in outpatient settings as appropriate\nPlaces and assists in maintaining the appropriate vascular access device throughout all hospital services except Pediatric patients; or refer Interventional Radiology when appropriate\nObtains informed patient consent for PICC placement\nPerforms ultrasound-guided placement of vascular access devices under sterile or surgical aseptic technique as appropriate\nAdministers local anesthesia(lidocaine) during ultrasound-guided procedures as appropriate\nUtilizes PICC tip confirmation system to confirm placement of PICC line\nFollows up on Chest X-Ray to confirm PICC placement of PICC line\nDocuments placement of vascular access devices in the electronic health record\nAddresses and assists medical and nursing colleagues in troubleshooting complications of vascular access devices\nFollows up on complications encountered during the PICC placement and provide recommendations for unsuccessful PICC placement\nDeclots VADs or assist staff as a resource in declotting\nProvides patient reaching on Vascular access device care and maintenance\nProvides resource education to staff on vascular access devices\nAssist in writing policies and procedures related to VAD insertion, care and maintenance\nProvides staff education on VAD care and management and complication management\nProvides physician education and serve as an expert consultant to physicians on VADs\nProvides education and training on ultrasound-guided placement of vascular devices to nursing and medical colleagues\nCollects VAD data and report on number of procedures performed independently, number of complications and rate of central line-associated blood stream infections\nParticipates in the development of internal quality improvement process as assigned\nMaintains knowledge and competency level related to infusion therapy\nEvaluates new VAD products introduced to the hospital and assists in training staff as appropriate\nWhy join us\nWe believe that investing in our employees is the first step to providing excellent patient care.
