Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW) is required

Outpatient role serving palliative care patients primarily in MaineHealth's Mountain Region (in-person and telehealth).Can be based locally, or out of our outpatient clinic in Scarborough.

Position Summary

The Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team which delivers patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize the quality of life and align the individual's medical care with what matters most to them. Key responsibilities of the Palliative Care SW include evaluating the psychosocial needs of patients and families, providing targeted counseling including anticipatory grief support, facilitating family meetings and goal-setting discussions, making referrals to community resources, and engaging in effective interdisciplinary team collaboration. This is primarily an outpatient role serving patients in MaineHealth's Mountain Region either in-person at primary care offices or via telehealth. This role may also provide care to patients and families in the Scarborough palliative care clinic. We offer mileage reimbursement for work-related travel using your personal vehicle.

This is a position for the professional social worker that involves the evaluation of psychosocial needs of patients / clients and their families, and provision of diagnostic / preventive and treatment services including individual, family, and group therapy. Also provides complex discharge planning, coordinates aftercare services, and makes referrals for other community services based on the treatment plan. The employee works in collaboration with other health / mental health professionals and provides consultation to the treatment team regarding patient / family social and environmental needs. Work is performed under general supervision in accordance with institutional and social work / clinical counselor policies.
