We are seeking a dedicated Licensed Professional Counselor (LPC) to join our multidisciplinary healthcare team. In this role, you will deliver high-quality behavioral health support, ensuring seamless care for our patients.
What You'll Do :
Therapeutic Interventions : Provide evidence-based therapy focusing on behavior modification, coping strategies, and emotional health. Assist patients in processing trauma and building resilience.
Integrated Care Collaboration : Collaborate with physicians, care managers, and primary care staff to create and maintain comprehensive care plans. Offer insights into patient needs and emerging behavioral health trends.
Patient and Community Support : Conduct screenings and assessments, and guide patients and families in accessing community resources and referrals. Provide crisis intervention when needed.
Documentation and Confidentiality : Maintain accurate, detailed records of patient care in the EMR, ensuring confidentiality and adherence to professional standards.
Qualifications :
Masters degree in Counseling or related field.
Active LPC license in the state of Michigan.
Experience in behavioral health therapy and patient care.
Strong communication and collaboration skills.
