Macas Home Care LLC, a residential service Agency in Maryland , is seeking motivated and experienced Registered Nurse, Physical therapist, occupational therapist, Social workers, dietician / nutritionist, audiologist, respiratory therapist  and nutritionist, to join our team and contribute to our mission of providing exceptional home care services throughout the greater Maryland and Washington, D.C. area.

These positions will support clients primarily in the Maryland and surrounding locations.

Position Overview :

The Staff  will be responsible for providing comprehensive nursing care to clients in their homes, ensuring adherence to established clinical standards and regulations. This role requires excellent clinical skills, strong communication abilities, and a commitment to patient advocacy.

Key Responsibilities :

  • Conduct initial patient assessments and clearly articulate the scope of services offered.
  • Develop comprehensive start-of-care assessments and complete all required onboarding documentation for new patients.
  • Design individualized plans of care (POCs) based on patient needs and clinical best practices.
  • Provide detailed instructions and training to assigned care team members regarding patient-specific needs and the established plan of care.
  • Monitor and report patient progress to relevant parties, including physicians, family members, and other healthcare professionals.
  • Maintain consistent communication and build strong relationships with clients, acting as a dedicated advocate for their well-being.
  • Cultivate and maintain relationships with community partners and referral sources to expand service reach.
  • Maintain open communication with Clinical Leadership with  the Office Administrator regarding patient conditions, medical changes, and any pertinent issues.
  • Review caregiver documentation to ensure accuracy and compliance with established protocols.
  • Actively participate in interdisciplinary team meetings to ensure holistic patient care and address comprehensive needs.
  • Fulfill other duties as assigned by the Administrator and as outlined in the job description.

Requirements :

  • Active Maryland (MD)  license in the trained profession  - Required.
  • License that allows coverage in both Maryland (MD) and Washington, D.C. - Preferred.
  • Minimum of 6 months  of experience in individual  roles
  • Minimum of an Associate's Degree in your profession.
  • Demonstrated ability to work collaboratively within cross-functional teams.
  • Excellent communication skills, both verbal and written, with the ability to effectively communicate with all levels of the organization.
  • Strong presentation skills.
  • Reliable transportation for home visits with mileage reimbursement provided.
  • Proficiency in computer / iPad technology for documentation and communication purposes.
