Job descriptionDeliver in-home direct patient care following the prescribed plan from the provider while monitoring progress and recovery closely. Evaluate and document patient conditions, reporting any changes as necessary (vital signs, blood / stool / urine, glucose, etc.). Administer medications, insulin, and IV fluids while maintaining thorough documentation. Examine and care for wounds, changing dressings and assisting with personal hygiene needs. Assess patients for potential injuries, infections, or bedsores and provide appropriate treatments. Collaborate with healthcare providers, aides, and allied health professionals to achieve exceptional patient outcomes. A current, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the applicable state. Reliable transportation to and from care sites and / or work locations. Experience with trachs and / or ventilators is preferred but not mandatory.
