As the 1115 Waiver Enhanced Navigator for Catholic Charities of Delaware, Otsego, & Schoharie Counties, you will play a pivotal role in individuals' lives, making a significant impact through :

• Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver

using the approved ACH screening. Ensure all demographic information is accurately collected and member consent is properly documented.

1115 Waiver Enhanced Navigator Minimum Salary : $22.00 / Hour

1115 Waiver Enhanced Navigator Maximum Salary : $22.00 / Hour

1115 Waiver Enhanced Navigator Schedule : Monday – Friday 8 : 30AM – 4 : 00PM (35 hours per week)

Qualifications :

Associates Degree in Human Services or related field

1-3 years of experience in mental health, substance use, or human services
