Job Title : Clinical - Senior LTSS Service Care Manager (RN)
Location : TX (Central SDA)
Shift : Monday - Friday (8am - 5pm)
Duration : 6 months, with possibility of assignment extension or conversion.
- RN will conduct F2F visits or work on tasks for the Central SDA.
- Candidate must live in the associated SDA for field visits 3-4 times / week.
- Mileage will be reimbursed.
- Perform 3-4 in-home visits with members / week to complete the Waiver Upgrade Assessment.
- Complete documentation & assist in coordinating services for the member.
- Coordinate with MD offices, clinics, DME companies, & other providers for member appointments, services, supplies, and equipment.
- Travel for member visits averages 1-2 hours one-way; visits typically take 2-3 hours.
- Fast-paced role requiring excellent multi-tasking skills and quick learning.
- Team interaction primarily through telephone calls, Microsoft Teams, email, and video.
- Remote training, except for field visit training with a team nurse; some in-person office training may be required.

Nice to Haves :
RUG or PDPM Certification4-6 years of experience working with the adult Aged, Blind, or Disabled population.Prior experience as a Care Manager or Service Coordinator for a Managed Care Organization, or similar.Previous experience working remotely (training is remote)Performance Indicators :
Complete at least 3 visits per week.Audit Score of 90% must be met.Best vs. Average :
RUG Certification preferred; required if not already obtained once hired.Candidate Requirements :
Required : Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience.Preferred : Bachelor's degree in Nursing preferred.Required : Registered Nurse, TX.
