Job Summary
Under the clinical supervision of a Licensed Clinical Social Worker (LCSW) who has completed a board-approved supervision course and has a minimum of three years of experience, or a Licensed Professional Counselor (LPC) with an Approved Clinical Supervisor (ACS) credential, the Telehealth Clinician serves as a key member of the treatment team. This role is responsible for providing mental health counseling, screenings and assessments, psychotherapy, and care coordination in collaboration with Eva's Village's Psychiatrist and / or Advanced Practice Nurse (APN). The Telehealth Clinician will work closely with Eva's Village Integrated Service Team, including Clinical, Medical, and Residential staff as well as other relevant personnel to ensure comprehensive care coordination and appropriate service linkages for clients.
Responsibilities
- Completes comprehensive screenings and assessments of clients if indicated and ensures treatment of clients with co-occurring disorders is provided by appropriately trained and qualified clinical staff (N.J.A.C.-10 : 161B-1.8(d)5., N.J.A.C.-13 : 34-10 2, and N.J.A.C.-13 : 34-10.3).
- Completes Clinical Assessments, consultations, and psychotherapeutic counseling services under the supervision of a State Licensed Clinical Social Worker (N.J.A.C. 13 : 44G-8.1(2) or other clinical supervisors deemed as appropriate by the Committee under N.J.A.C. 13 : 34C-6.2 and 6.4 or the supervision of a qualified supervisor as defined in N.J.A.C. 13 : 34-10.2).
- Completes the professional application of Social Work methods, Professional Counselor values in the assessment and psychotherapy counseling of individuals, families, or psychotherapy groups (N.J.A.C. 13 : 44G-1.1, N.J.A.C. 13 : 34-11.2, N.J.A.C. 13 : 34C-2.1).
- Engages in full compliance with the Scope of Practice of an LSW and LAC, including Social Work Services, Clinical Social Work Services, and Professional Counselor / Associate Counselor under the supervision of an LCSW or LPC (N.J.A.C. 13 : 44G-3.2, N.J.A.C. 13 : 34-10.1, and N.J.A.C. 13 : 34C 1.2-1.2).
- Completes detailed assessments and / or evaluation of the client's history of psychological and / or psychiatric treatment (e.g., previous admissions to psychiatric facilities, history of suicidal / homicidal ideation and attempts, outpatient psychiatric treatment, psychotropic medications, etc.) (N.J.A.C. 10 : 37E-2.3).
- Completes DSM-5-TR diagnoses, Individualized Service Plans (ISP), level of care recommendations, and service pre-authorizations as indicated.
- Completes the client service plan by the fifth session or within 60 days of the first face-to-face visit, whichever occurs first, unless clinical circumstances dictate otherwise (N.J.A.C.-10 : 37E-2.4).
- Provides service plan reviews at significant decision points in each client's course of treatment, documented in progress notes or treatment plan revisions. Significant decision points include transfers, discharges, changes in medication, and any significant change in the client's condition or situation (N.J.A.C.-10 : 37E-2.4).
- Reviews the service plan with the client at least every three months for the first year of treatment and at least every six months thereafter (N.J.A.C.-10 : 37E-2.4).
- Provides Co-Occurring individual counseling to clients.
- Provides detailed daily progress notes for all individual, group, and / or family counseling services in the client's EHR at the close of business day.
- Performs case management duties, including making referrals and coordinating scheduling of evaluations and follow-up appointments with Eva's Village psychiatrists and / or APN or other community psychiatrists / behavioral health providers.
- Participates in clinical staff meetings within the agency as applicable.
- Maintains effective intra- and interdepartmental communication.
- Participates in case conferences with MDT members as clinically needed to determine the best course of treatment and address all client needs as per the treatment plan.
- Maintains client Electronic Health Record (EHR) and ensures up-to-date client documentation for mental health services provided.
- Maintains professional licensing requirements per the state of New Jersey.
- Other projects / activities as deemed necessary by the Coordinator of Mental Health Services and / or Sr. Director of Integrated Services.
Qualifications
EDUCATION : Required : Master's degree in mental health counseling or related field (MSW, MA, or MS) with one of the following mental health licenses (LAC or LSW, LCSW, or LPC) and / or substance use disorder licensure (LCADC).
EXPERIENCE : Minimum one to three years' experience in the Behavioral Health field with at least two years' experience in assessment, crisis intervention, and facilitation of individual and group sessions.
Knowledge of the bio-psychosocial model, DSM-5-TR Mental Health Disorders and Diagnosis; Knowledge of resources for Mental Health Treatment and recovery support services in Paterson, NJ and Passaic County; Knowledge of ASAM criteria; Understanding and experience of substance use disorder treatment; Understanding confidentiality rules and regulations related to Mental Health Disorders; Ability to speak, read, and write English.
