Missouri Licensed Case Manager RN

A company is looking for a Case Manager, Registered Nurse (Remote).

Key Responsibilities

Engage with members telephonically to coordinate their care and develop individualized care plans

Provide disease management education and support, ensuring collaboration with the interdisciplinary care team

Document care management activities accurately and participate in team meetings to discuss member status

Required Qualifications

Active and unrestricted Registered Nurse (RN) licensure in Missouri or compact licensure in state of residence

Proficient in Microsoft Office Suite and comfortable using technology for collaboration

Access to a private space for remote work

3+ years of nursing experience and 2+ years in case management or home healthcare coordination

Associate's of Science in Nursing (ASN) degree required
