What You'll Do:

• Plan, coordinate, and implement personalized care plans
• Administer medications and treatments in accordance with physician orders
• Maintain accurate documentation and follow infection control procedures
• Work collaboratively across teams to ensure a safe, supportive environment
• Respond to resident needs with compassion and professionalism

What You'll Need:

• Licensed Practical Nurse (LPN) with valid Illinois license (required)
• One to two years of experience in a long-term care environment (preferred)
• Graduate Registered Nurse (GRN) will be considered if licensure is received within 45 days of employment
• Strong interpersonal and communication skills, both verbal and written
