Breaking Ground operates nearly 4,000 units of housing across New York City, as well as in upstate New York and Connecticut. Our supportive housing combines affordable housing with services designed to help people maintain their homes long-term. This approach is recognized as an effective solution to chronic homelessness.
Beyond providing housing, our programs and services assist street homelessness, especially for those who have been homeless the longest. Through our Street to Home outreach and transitional housing resources, we help individuals secure and sustain permanent housing.
The Community Health Specialist, reporting to the Assistant Director of Programs, provides on-site and field assessments for the Safe Options Support (SOS) outreach teams in Staten Island. This role involves engaging with individuals to assess homelessness history, identify needed services, and determine suitable placement options. The specialist ensures the use of Harm Reduction and Housing First strategies, along with other best practices, to deliver high-quality care. The role includes managing a caseload and maintaining client records in compliance with regulatory and agency standards.
The position primarily operates during business hours (Monday-Friday), with availability for off-hours emergencies or special projects.
