About The Position

We are seeking an experienced Registered Nurse Case Manager to join our healthcare team. The Case Manager will be responsible for providing comprehensive care coordination, discharge planning, and utilization management to ensure quality patient outcomes and cost-effective care.

Job Requirements

Requirements :

License : Active Registered Nurse (RN) License – Required

Experience : Minimum 2 years of recent acute care hospital experience as a Case Manager – Required

Education : Bachelor of Science in Nursing (BSN) – Preferred / Required based on facility

Certifications :

Basic Life Support (BLS) – Required

ACM (Accredited Case Manager) or CCM (Certified Case Manager) – Preferred
