Job Description
The RN Case Manager monitors each child’s health and medical treatment, provides counseling and training to foster parents, coordinates services with outside medical providers, and ensures compliance with all health-related documentation and regulations. The nurse will make monthly home visits , attend specialty clinic appointments , and maintain accurate, up-to-date records for each child in care.
Key Responsibilities :
- Provide ongoing health monitoring, assessment, and follow-up for children in care
- Deliver training and education to foster and biological parents on health practices and medication administration
- Coordinate medical appointments, specialty care, and follow-up with outside agencies and healthcare providers
- Communicate regularly with families / guardians regarding medical conditions, treatments, and appointments
- Participate in community visits including hospitals, emergency rooms, and home visits as needed
- Obtain and maintain accurate health documentation (immunizations, birth records, health assessments, etc.)
- Ensure full compliance with all internal, external, and regulatory health requirements
Qualifications :
Registered Nurse (RN) license – New York State (required)Prior experience in pediatrics, community health, or case management preferredStrong organizational and communication skillsCompassionate and collaborative approach to working with children and families
