About the Role

You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include :

• Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day
• Collaborating with interdisciplinary teams to optimize patient outcomes
• Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
• Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Expected compensation is estimated base salary range of $98,126 to $116,152 annually with no cap on productivity income potential. This full-time role is benefit eligible.
