Must-Have : Experience in acute hospital care coordination and discharge planning.
Description :
The registered nurse will work within a team of case managers.
The focus will be on coordinating care within an acute care facility.
The role will involve floating to different units as needed, including emergency department, intensive care unit, pediatrics, and specialty units.
Applicants must be willing to work in various areas of the hospital.
Floating to another nearby campus may also be required.
Weekends are usually required, typically every other week.
Experience in discharge planning and care coordination is essential for this position.
Behavioral health experience does not count as acute care hospital experience.
Charting experience in EPIC is preferred.
Candidates must have two supervisory references covering one year of employment in a comparable specialty.
All travelers are expected to float between various like units within their skill set.
Compliance with communication device provision for job performance is necessary.
Travelers must commit to working three out of the five major holidays each year.
