Licensed Practical Nurse (LPN) – Assertive Community Treatment (ACT) Program

Location : Glen Cove, NY (Nassau County)

Schedule : Full-Time | Monday–Friday, 9 : 00 AM–5 : 00 PM

Compensation : $35–$37 per hour

We are seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team. This is a field-based role where you’ll work as part of a supportive, multidisciplinary team providing mental health and wellness services directly to individuals in the community.

Key Responsibilities :

Deliver mobile, community-based nursing care as part of a team supporting individuals with serious mental illness.

Support recipients in building life skills, improving self-care, and managing health-related goals.

Collaborate with ACT team members and assist medical staff in coordinating health and psychiatric care.

Develop and implement wellness-focused interventions based on OMH’s Wellness Self-Management (WSM) model and other best practices.

Advocate for clients, helping them access community resources and maintain stability in the least restrictive environment.

Requirements :

Active NYS Licensed Practical Nurse (LPN) license

Valid NYS driver's license and access to a reliable vehicle (travel required throughout Nassau & Suffolk Counties)

Experience or interest in behavioral health nursing, community nursing, or wellness promotion

Minimum of one (1) year of experience in mental health, substance use treatment, or health and wellness education preferred
