JOB DESCRIPTION

An employee in the Southern CA area is seeking an RN Case Manager. This position is fully remote, but candidate MUST sit in Southern California area . Must have unrestricted CA RN License. The RN Case Manager will help identify, coordinate, or provide appropriate levels of care for patients. Responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting.

SCHEDULE :

• Monday-Friday, 8 : 00 am-5 : 00pm
• 4-month contract, with the potential to extend / convert
