POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Floating among sister campuses required

MINIMUM REQUIRED QUALIFICATIONS – FL / compact RN license; BLS; 2+ years acute hospital case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday-Friday 8a-4 : 30p (40hrs / wk)
